What to Expect

The first stage of your Renuvion treatment journey will be a consultation with one of our expert surgeons.

At your consultation, your surgeon will go through the Renuvion technique including the use of helium plasma and radiofrequency energy to tighten and rejuvenate the skin.

Your will evaluate the areas you wish to treat, assess your for the procedure, and advise what results can be achieved. Your surgeon will then discuss:

Following your consultation, you will be introduced to your dedicated patient coordinator who will be your point of contact should you wish to proceed with Renuvion surgery.

In the interests of safety, all our patients are assessed against our patient criteria. Following your consultation and review of your past medical history, the physician or preoperative nurse will choose any pre-operative tests considered to be appropriate, most of which are routine blood tests.

You will also be advised on any precautions you will need to take in the days or weeks leading up to your Renuvion procedure.

We recommend that patients that smoke, refrain from smoking 2 weeks before they are due to undergo Renuvion.

Renuvion treatment is typically performed under local anaesthetic with sedation. The procedure will take place at one of our dedicated in-clinic theatres or private hospitals.

Before the procedure begins, your surgeon will take before and after photos and draw markings on your body on the areas that are being treated.

You will then lay down on the surgical bed and our medical team will ensure you are feeling comfortable before starting the procedure.

Tiny incisions (similar to those used for liposuction) are made, through which the Renuvion device is inserted beneath the skin. The Renuvion device is wand-shaped for precision and uses helium to deliver and drive the radio-frequency energy into the exact place of the wand placement. From there, it heats the treatment area to force cells to contract. The heat is then cooled within a fraction of a second, making it very safe for the surrounding skin and tissues. This unique technique means that a more focused heat target can be achieved in comparison to other similar skin-tightening procedures.

Once the procedure is complete, bandaging will be applied to the incisions and compression clothing will be put on which patients are required to wear for 2-4 weeks depending on the treated areas and if the procedure was combined with Vaser Liposuction.

The effects of the local anaesthetic used during Renuvion often lasts for many hours following the procedure but as this starts to wear off, you may experience some discomfort which can be with pain medication.

Renuvion downtime will vary depending on the areas treated and if it was combined with Vaser Liposuction.

Bruising is to be expected for around 7 days following the procedure. You will have small scars around the incision sites which will fade over time.

A compression garment should be worn for the amount of time as requested by your surgeon, this can be anywhere from 1-2 weeks. If combined with Liposuction, this will be longer.

Patients can resume exercise within 1-2 weeks following their Renuvion procedure. If performed with Liposuction, Masseter (Jawline) Botox we advise that strenous exercise is avoided for 4-6 weeks as directed by your surgeon.

A follow-up appointment with your surgeon will be made for 6 weeks after your procedure. During this appointment, your surgeon will assess your results and provide further advice on how to continue with your recovery process and how to best maintain your results.

The procedure results in a contraction of the soft tissue that is noticeable immediately after the procedure. The results will continue to improve over the next 6-9 months as the body goes through the healing process.

Correcting Dermal Filler Complications

Dr Anna Hemming recounts how she handled a rare & particularly challenging complication

At 1.42 pm, on a Thursday lunchtime, the notification of an email arrived on my screen. As I was patients I saw the first few words:

I didn’t want to bother you, but I thought I would check, is this normal?

 

Normally, I would leave my experienced team to deal with all patient emails, however, this was a patient I had treated with the day and, knowing the patient, something within the email didn’t seem right. Moments later, I was on the phone with her, asking if she was in pain (no), whether there was any blanching (yes), and various other questions. A photo immediately arrived of the kind we have all seen at complications training. This was not normal, and we needed to bring her in. Being 90 minutes away from the clinic, she arrived as soon as she possibly could.

In the meantime, the clinic ran as normal, patients were seen, and, in the back of my mind, my complications file was being pulled out and the algorithm for vascular (VO) ran through. By the time the patient arrived at the clinic, I had reviewed her notes (after images were normal, no mottling and no altered capillary refill time (CRT), reviewed the ACE guidelines for VO, and had all the emergency drugs at hand, just in case.

My patient is a 42-year-old with asymmetry. I had treated her 12 months previously with dermal filler with great success. Her 12-month review had recently passed and there was distinct volume loss to the temple, medial and lateral suborbicularis oculi fat (SOOF), as well as the tear trough. Her left side was always more depleted than the right and we had a plan to stabilise the deep fat pads, bringing deep alignment and then review, to address the tear trough depressions.

At the review, the tear trough filler was used to lift the under-eye, especially on the left. The results were lovely, there was no pain or unusual after-effects, until seven hours after the filler, when the patient noticed some numbness (she thought initially it was the local anesthetic from the treatment).

In the evening, the area was slightly pinker, but it wasn’t until the next day and 24 hours after treatment that she emailed, as the area was still a bit pink.

HOW TO ASSESS POTENTIAL VO

Patients are often in pain, have reduced CRT in the area and surrounding skin, and display pallor initially and then mottling.

Rapid action is necessary to reverse the hypoxia before necrosis establishes, leading to tissue breakdown and wounds.

 

In this patient, the pallor stage was not visible in clinic, presentation occurred at 24 hours in the livedo reticularis phase.

Phases of a VO

1. Pallor – Occurs with immediate blockage of an arteriole as the blood flow is interrupted and blocks tissue perfusion. Lasts seconds – or persists longer.

2. Livedo reticularis – A mottled pattern appears on the skin from the build-up of deoxygenated blood from the venous network. Can occur rapidly, lasting hours.

3. Pustules Typically at 72 hours due to the reduction in pH and sweat, along with metabolic changes due to hypoxia allowing staph. aureus bacterial .

4. CoagulationIndicating necrotic change and can occur before pustule formation. Caused by worsening hypoxia, the skin darkens as cell lysis occurs and there is a leaking of blood into the tissues. Skin tissue remains firm due to the coagulative necrotic process.

5. Tissue destruction – Skin breaks down due to a of denatured structural proteins (collagen, fibrin, elastin) neutrophils, bacteria, and haemoglobin. Devitalised tissue is initially moist creamy/yellow or green (slough) and then becomes black (dark) and dry. This occurs days after the occlusion.

HOW TO TREAT A VO?

• Stop treatment (if they are with you) and inform them about what is happening

• Check and video CRT on both affected and unaffected skin for comparison

• If CRT is delayed, it indicates vascular compromise

Massage the area firmly, applying heat to encourage vasodilation

• Assess

• Get help

Hyaluronidase (do not skin test, ensure anaphylaxis medications are at hand just in case)

Disinfect the skin

Reconstitute 1500 hyaluronidase in 1ml NaCl 0.9% or 1-2% lidocaine

Infiltrate 1500IU by needle or cannula throughout the affected artery and wider area of ischemia. More than one vial may be needed

• Apply heat and massage area vigorously (helps mechanical breakdown of HA)

• Assess CRT and if >3 repeat hyaluronidase hourly

• Review patient daily

Clinical resolution may be over the following days to avoid deterioration

• Make detailed notes and take images and videos

• Advise insurers so they are aware of the situation.

Medications that may help Aspirin or Clopidogrel 300mg stat and 75mg per day.

The may also help compromise:

Nitroglycerin paste

Hyperbaric oxygen

Steroids only if clinical indication

• Wound management

Antivirals if tissue has started to break down

Antibiotics.

PROGRESS OF THIS PATIENT’S VASCULAR EVENT

On arrival in clinic the day after dermal filler treatment, we talked through the situation openly. She was not in pain; her CRT was sluggish in the area treated and the surrounding vascular pathway. Livedo reticularis was present with non-blanching erythema and even greying of the tissue in the distal vascular pathway.

My gut feeling was the vessel had experienced a spasm, affecting the distal branches delivering oxyhaemoglobin to the skin.

With open discussion we planned her . Immediate aspirin, hyaluronidase and antibiotics were started due to the delayed presentation, to try to decrease pustule formation and .

Day two

As I was attending a conference 10 away from her the day, we to review at the conference, where I arranged a private room and place where we could treat her again. 1500IU of hyaluronidase was administered, exosomes were started topically and after consulting with colleagues a short course of prednisolone commenced.

Day three

We arranged hyperbaric chamber sessions starting the following day along with review and a further 1500IU as the area was still firm. Tiny white pustules started to appear in the apical triangle to the side of the nose. The erythema was and the numbness was improving.

Day four

The area was injected one last time with 1500IU hyaluronidase and a further hyperbaric chamber session attended. Bruising from hyaluronidase flooding can be seen in the filler treatment area.

Day five

A small area in the apical triangle has potential for breakdown.

Day seven

The patient has a further hyperbaric chamber session. The bruising, inflammation and vascular compromise settled and the apical triangle crusting was mildly better.

Day 12, 16 and 45 saw huge improvements in the look and feel of skin, with reduced numbness. The patient was left with a small amount of erythema. The apical triangle remained intact and didn’t breakdown.

• 9 appointments

• 4 x 1500 IU hyaluronidase

Aspirin 300mg stat, 75mg OD

Flucloxacillin 500mg QDS 7/7

Prednisolone 40mg OD 5D

• 5 chamber sessions

We have our next review planned and aim to help resolve the erythema in with laser genesis or excel V+ treatment.

The patient is hugely relieved that we were able to get on top of the vascular event as soon as we were aware of it. She is happy with our treatment.

Haemorrhoids Injection Treatment Information

Sclerotherapy entails injecting a solution into the tissue around each haemorrhoid. The solution normally contains chemicals including zinc chloride, quinine, and polidocanol. The goal is to damage the blood arteries that feed haemorrhoids, causing them to shrink.

Doctors may sclerotherapy for the treatment of grade 1 or grade 2 haemorrhoids. This technique reduces blood flow to the haemorrhoids by destroying blood vessels. It is to repeat the procedure every few weeks to treat all of the enlarged haemorrhoids.

The doctor will determine the most appropriate therapy by the size of the haemorrhoids and the intensity of the symptoms. Each method has advantages and disadvantages. When a person develops grade 3 or grade 4 haemorrhoids, our surgeon may recommend prescribe a more radical solution such as Rafaelo radiofrequency treatment. We also offer this at our Harley Street clinic.

The best place to start is a consultation and proctoscopy scan.

The (Radio Frequency Ablation of Haemorrhoids under Local Anaesthesia) is a procedure that uses radiofrequency ablation to treat internal haemorrhoids. This is a clinically proven solution with a very high success rate.

Banding is another suitable for smaller haemorrhoids. Surgeons effectively tie a rubber band around the base of the haemorrhoids to block blood flow to the haemorrhoid. After about 3-5 days, it shrinks and dies. Without your knowledge, the dead tissue and rubber band slip off in the stool. This technique may need to be repeated after a few weeks to cure all of the enlarged haemorrhoids.

The best way to identify haemorrhoids is through a physical examination and scan. By inspecting the area surrounding your anus, our specialist can identify external haemorrhoids. Your doctor may also do an internal exam and/or proctoscopy scan to examine inside your anus to detect internal haemorrhoids.

Patients who require haemorrhoid injections may find that the cost is more affordable than they’d expected Typically, the cost is around £625.

You should be able to go home after the treatment and return to work the following day unless otherwise instructed. There is no overnight stay after this procedure.

Reduce recurring haemorrhoids by avoiding constipation and not straining during bowel movements, exercising, staying hydrated, and the quantity of fibre in your diet.

Haemorrhoids are less expensive than Rafaelo, but they don’t offer such a high success rate. Therefore you may consider radiofrequency treatment with a greater chance of success. This, however, is dependent on the condition of your haemorrhoids.

Sclerotherapy is a minimally invasive procedure used to treat internal haemorrhoids. A chemical solution, called a sclerosant, is injected into the tissue around the haemorrhoid. This causes the blood vessels to shrink and the haemorrhoid to shrivel up and eventually fall off.

The injection itself is generally not painful because it is administered into an area with very few nerve endings. You might feel a slight pressure or a brief, mild sting. A local anaesthetic is used to minimise discomfort.

Common side effects are generally mild and may include a feeling of fullness in the rectum, mild bleeding, or a slight ache. In rare cases, more serious side effects can occur, such as infection, allergic reaction to the solution, or urinary retention. You will be made fully aware of any risks prior to having the treatment.

Sclerotherapy is an effective for many people with internal haemorrhoids and is especially effective on small, bleeding haemorrhoids. However, it may not be a permanent solution, and haemorrhoids can sometimes return. Repeat treatments may be necessary.

Alternative treatments such as radio frequency () generally offer the highest success rates.

The procedure is very quick and can often be done during a short clinic visit. It typically takes only a few minutes to complete. The consultation inevitably takes longer, as our Colorectal Surgeons take time to properly assess and diagnose before offering treatment.

The number of treatments varies depending on the size and number of haemorrhoids. Some people may only need one session, while others may require several treatments spaced a few weeks apart to achieve the desired result.

Recovery is usually very quick. Most people can return to their normal activities immediately after the procedure. You might experience a sensation of fullness or a dull ache in the rectum for a day or two.

This treatment is typically recommended for people with small to medium-sized haemorrhoids (Grade 1 and some Grade 2). It is not usually effective for larger haemorrhoids (Grade 3 and 4) or external haemorrhoids.

7 Things You Should Never Do To Your Skin

Picking and touching your skin increases the risk of developing bacteria, scarring and infection. According to it’s very important to keep your hands away from your face throughout the day to avoid spreading oil, dirt and bacteria from your hands which can skin conditions such as acne, blackheads, clogged pores and breakouts.

Also try to keep your nails clean as they are a breeding ground for germs and bacteria too!. Sometimes it is hard to not touch your face so make sure your hands are kept clean, carry anti bacterial gel if you feel this will help.

it’s sunny, raining or even snowing, it’s important to. It will protect your skin from UV lights as 80% of the sun rays can pass clouds. This means even during the winter months when most people would not think of using it make sure you are protected.

Legally companies are not allowed to advertise their cream as being over a factor 50.  Make sure you wearing the highest factor SPF that is right for your skin type. Wearing sunscreen daily helps to keep the skin protected from UV rays which is one of the most common reason clients get , Melasma, sun spots, uneven skin tone, dark spots and dark lines on their face and also body.

Exposing your skin to sun without SPF also promotes ageing of the skin.  Sun damage from UV rays causes photo ageing of the skin which leads to discolouration and fine lines, wrinkles and sagging. The best thing you can do to protect your skin from being wrinkled later on in life is to apply the SPF so get your suncream on!.

Smoking and drinking can be one of the to your skin as it promotes a fast acceleration of your skins natural ageing process. Smoking causes thinning of blood vessels throughout the body which limits the supply of oxygen and nutrients to your skin which maintain a healthy and fresh looking skin.

Drinking dehydrates the skin. Over time, the skin slowly damages and results in a loss of and elastin which results in wrinkles, dull skin tone, dark patches and smoker lines caused by smoking around the mouth area. If it is a big step to give up then reducing the amount you smoke and drink will definitely benefit your skin.

Excessive sugar, additives, preservatives, oil and salt are found in processed foods. When we consume processed foods which are packed with excessive ingredients in our diets it promotes degradation of collagen in the skin which can cause your skin to develop conditions such as which aggressiveness leads to breakouts and oily skin.

Take a look at your diet and try to replace processed foods with natural ingredients where possible. We know that sometimes eating processed food is unavoidable but try planning ahead, like taking a packed lunch to work or go for healthier fresher options when you are eating out.

This includes blackheads caused by oil and dirt gathered in your skin and which develops by not exfoliating the dead skin cells which does not allow the skin to renew and develop skin cell turnover.

Acne is another cause that may happen to your skin when you forget to exfoliate, one of the main reasons why we suffer from acne is dead skin and . By exfoliating we our skin to renew and replenish and promote a new skin for a healthy and clean looking skin.

Make exfoliating as part of your weekly skin care routine. There are great products available online or on the high street that will do a good job with prices starting from only £5. Its money well spent!. If you want to really deep cleanse your face and leave feeling a million dollars then you can always look into having a or other clinic led treatments.
