Lipotherapy or Injection Lipolysis treatment involves the use of an injectable to break down stubborn areas of fat, which are then eliminated through the body’s own lymphatic system. This treatment area has been controversial at times, with much talk about safety and efficacy of the various products on the market. Lipostabil®, also marketed as Flabjab™, Lipomelt, Liposdissolve and Fat-Away, which contains phosphatidycholine has being unlawfully advertised in the UK as a cosmetic product for the reduction of fat since 2004. Lipostabil® is not licensed in the UK. AQUALYX™, introduced to the UK in 2013, is an injectable treatment of deoxycholate solution which is being marketed for ‘Aqualysis’ or ‘Aquaplasty’ treatment as a means of treating localised pockets of fat underneath the surface of the skin. This is a European CE Marked Medical Device.

The treatment of Lipotherapy or Injection Lipolysis, whereby a substance is injected into pockets of fat in order to reduce them by causing their destruction and elimination by the body has been a highly controversial one, with much talk about safety and efficacy of the various products on the market. 

One such treatment, Lipostabil®, which was also marketed as Flabjab™, Lipomelt, Liposdissolve and Fat-Away, was being unlawfully advertised in the UK as a cosmetic product for the reduction of fat over a decade ago. Please see below for more information on the issues with this product.

, introduced to the UK in 2013, is an injectable treatment of deoxycholate solution (containing deoxycholic acid) which is being marketed for ‘Aqualysis’ or ‘Aquaplasty’ treatment as a means of treating localised pockets of fat underneath the surface of the skin. It dissolves or liquefies fat cells once injected and the resulting fatty acids can then be excreted through the body’s lymphatic system. 

Lipostabil®, also marketed as Flabjab™, Lipomelt, Liposdissolve and Fat-Away, is being unlawfully advertised in the UK as a cosmetic for the reduction of fat.

Lipostabil® is licensed as an intravenous medicine in Germany, for the prevention and treatment of blood vessel blockages by fat particles (fat embolism). It is not approved or licensed for cosmetic use. Product safety has not been established for subcutaneous use for cosmetic purposes. Consumers should be aware of this should they be offered the .

The Medicines and products Regulatory Agency (MHRA) has received three reports of potential adverse reactions to cosmetic treatment with Lipostabil®. Some patients may experience allergic reactions or sensitivity issues to the product. The usual route of administration for cosmetic surgery is subcutaneous (injection under the skin) and this is warned against in the manufacturer’s information sheet which specifically states that Lipostabil® should not be given subcutaneously.

Lipostabil® is not licensed in the UK. However, it may be imported into the country by doctors for clinical use for the prevention and treatment of fat embolism subject to compliance with the Medicines ( Provisions for Licences and Certificates) Amendment Regulations (SI 1999/4). This allows for from elsewhere in the EU by holders of Wholesale Dealers Licences, or from outside of the EU by holders of Import Licences. When ordering, proof must be available from the doctor to demonstrate the existence of a special clinical need for the patient. The MHRA must be consulted about the import by the wholesaler using the notification procedure set out in SI 1999/4. The use of Lipostabil® for cosmetic purposes does not constitute a special clinical need.

Unlicensed medicines may not be in the UK and should not be imported for cosmetic purposes. The MHRA has concerns about the safety of this product for cosmetic use and continues to investigate a number of cases where medicines legislation may have been breached.

To Whom It May Concern,

It has come to the Agency’s attention that the product Lipostabil®, also marketed as Flabjab™, Lipomelt, Lipodissolve and Fat-Away, is being promoted as a cosmetic in the UK and is available for the reduction of fat.

Definition of a medicinal product

A relevant “medicinal product” is defined in Article 1 of 2001/83/EC as:

Any substance or combination of substances presented for treating or preventing disease in human beings. Any substance or combination of substances which may be administered to human beings with a view to making a medical diagnosis or to restoring, correcting or modifying physiological functions in human beings is likewise considered a medicinal product“.

Lipostabil

The product Lipostabil® is an intravenous preparation of phosphatidycholine and is for the prevention and treatment of blood vessel caused by fat particles (fat embolism). It is indicated as a medication for reducing blood fat (liquid reducer). It is a licensed medicine in some European Member States but not the UK.

The Medicines and Healthcare products Regulatory Agency (MHRA) has determined that Lipostabil® is a relevant medicinal product and subject to the of The Medicines For Human use (Marketing Authorisations Etc) Regulations 1994 as amended (Statutory Instrument 1994/3144). In accordance with section 3(1) of S.I. 1994/3144, no relevant medicinal product shall be placed on the market unless a marketing (formerly Product Licence) has been granted for it.

Circumstances in which an unlicensed medicinal product may be supplied.

In the UK, an unlicensed relevant medicinal product (other than a herbal medicine in respect of which different rules apply) may only be supplied in accordance with the provisions of Schedule 1 to S.I. 1994/3144. This amongst other things, that subject to certain conditions, an unlicensed product may be supplied in response to the order of a doctor or dentist, and on his personal responsibility, to meet the clinical needs of an individual patient.

The conditions are set out in paragraph 2 of Schedule 1. In very general terms there must be a bona fide unsolicited order. The relevant medicinal product must be formulated in accordance with the requirements of a doctor or dentist registered in the UK for use by his individual patient on his /her responsibility. It must be supplied to a doctor or dentist, or for use in a registered pharmacy, hospital or health centre. The relevant medicinal product must also be manufactured, assembled, imported or distributed by the holder of an authorisation.We would draw attention, in particular, to the fact that the regulations expressly provide that such products cannot be advertised to the general public or included in any catalogue, circular or price list.

Importation

Persons who source a relevant medicinal product from another Member State of the European area (EEA- all EC countries, together with Norway, Iceland and Liechtenstein) other than for personal use must hold a Wholesale Dealer’s Licence (WL). Persons importing a product from outside the EEA must hold a Wholesale Dealer’s Import Licence (WI) (Section 3 of SI 1994/3144).

A holder of a WL or WI license who imports an unlicensed relevant medicinal product must comply with the provisions of Paragraph 8B of Schedule 3 to the Medicines (Standard Provisions For Licences And Certificates) Regulations 1971 which provides that the holder of a WL or WI who imports an unlicensed relevant medicinal product for the purpose of sale and supply in accordance with schedule 1 of S.I. 1994/3144 must meet specified requirements. The requirements provide for notification to the Licensing Authority (LA) concerning the importation of the unlicensed relevant medicinal product, record and set maximum quantities for import.

The quantities permitted per importation are: 25 individual doses, or sufficient material for 25 patients in a 3 month period. Following receipt, the LA has 28 days to assess notifications.

Regulation of cosmetic products is governed by the Cosmetic Directive 76/768/EC. Lipostabil is not a cosmetic within the meaning given to that term by the Directive. The recitals to the Directive state expressly that:-

….products containing substances or preparations intended to be ingested, inhaled, injected or implanted in the human body do not come under the field of cosmetics;

In particular, importers of such products must provide MHRA with prior of import. Importation will only be permitted where the relevant conditions in Schedule 1 to SI 1994/3144 and Schedule 4 to SI 2005/2798 are met.

Importers from elsewhere in the European Economic Area (EEA) must be holders of Wholesale Dealers Licences (WL), those from outside the EEA must be holders of Manufacturer’s Licences, (ML) (previously Wholesale Dealers Import Licences WI, now being updated to Manufacturer’s Licences.

Q Can I bring this in for my own personal use?A
The regulations do not prohibit individuals from importing Lipostabil® for their own use. However, you should be aware that control of the distribution network from manufacturer to end user, including storage by the end user, is of major importance in ensuring the quality of medicines. Recommendations concerning storage temperatures given on product labels and in product literature are made to ensure optimum stability of products to which they relate in order to help to ensure that they remain safe and effective. For example, storage and transportation of medicines at temperatures above those recommended, can increase their rate of degradation. Failure to adhere to such requirements may result not only in reduced effectiveness, but also in increased toxicity.

MHRA considers that if a doctor is involved in, for example, recommending a supplier, assisting in ordering or storing the medicinal product, the product cannot be regarded as for personal use.

Q If I import this product for personal use into the UK, can I lawfully give it to anyone else?A
Someone who personally imports Lipostabil® for his own use may give it to someone else in certain limited circumstances, e.g. a family member. Determination of whether is for personal use will depend on the particular factual circumstances and is ultimately a matter for the courts.

Q I have seen Lipostabil® advertised in the UK – what is the situation with this?

Unlicensed medicines may not be advertised or marketed in the UK. In addition, it is not lawful to advertise a prescription only medicine to the public. Lipostabil is a prescription only medicine. The MHRA will investigate cases where either activity appears to have taken place, and may take regulatory action.

As the face ages, the temporal bone loses density, the fat pads shrink, and the temporalis muscle reduces in bulk, creating a visible hollow at the side of the face. Restoring this volume lifts the tail of the brow, reduces upper eyelid heaviness, and improves overall facial without surgery or significant downtime.

There is a tell-tale sign of ageing that most people cannot quite put their finger on. Patients sit across from me and say they look exhausted even when they are not, or that something has shifted but they cannot identify what. They have noticed , perhaps, or a little drooping around the jowls. But often, when I look at their face as a whole, the culprit is something far less obvious: their temples.

Temple hollowing is, in my view, one of the most underappreciated contributors to facial ageing. And treating it is one of the most rewarding things I do in clinic.

What Causes Hollow Temples?

To understand why temples matter so much, you need to think about the face not as a collection of individual features, but as a three-dimensional structure supported by bone, fat, muscle, and skin, all working in concert.

From our mid-thirties onwards, we lose bone density in the skull, including the temporal bone. The fat pads in the region, and there are several distinct of them, begin to shrink and descend. The temporalis muscle, which runs through this area, also loses volume over time. The result is a gradual caving inward of the area that sits between the outer edge of the eyebrow and the hairline.

What this does to the face is significant. When the temples hollow, the skin and soft tissue above the brow loses its support. The tail of the brow drops. The upper eyelid can start to look heavy or hooded. can appear less prominent, because the scaffolding above them has subtly collapsed. The face acquires a slightly skeletal quality, visible in profile, that reads unmistakably as age and fatigue.

It is worth noting that temple volume loss does not discriminate. I see it in patients in their late thirties, and I see it quite dramatically in patients who have lost weight rapidly, who exercise intensely, or who have very lean body compositions generally. In those cases, the hollowing can appear even before other signs of facial ageing become prominent.

Why Temple Hollowing Is So Often Overlooked

Most people seeking cosmetic treatments focus on the middle and lower face: the nasolabial folds, the lips, the . And I understand why; these are the features that tend to the most attention and cause the most distress. But treating the lower face in isolation, without addressing volume loss higher up, can produce that look slightly off without anyone being able to say precisely why.

Think of it like renovating a house from the ground floor up while the roof is sagging. You can polish the floors and repaint the walls, but structurally, something still feels wrong.

When I treat the temples before or alongside other areas, patients consistently notice that the whole face appears lifted, not just the temples themselves. The brow sits higher. The upper eyelid appears more open. Cheekbones look more defined. It is one of those treatments where the impact is quietly but profoundly transformative, and where patients are often surprised by how much difference a single, well-placed intervention can make.

This is also why I always assess the upper face carefully during consultations, even when a patient comes in asking about something else entirely. Temple hollowing is the underlying cause of concerns that patients attribute to other areas: hooded eyes, a heavy brow, general facial fatigue. Addressing it changes the whole picture.

How Temple Filler Treatment Works

Temple augmentation using is a well-established, safe, and effective when performed by an experienced and medically trained practitioner. I use acid fillers, a substance that occurs naturally in the body, to volume to the temporal region.

The key to good temple treatment is precision and restraint. This is not an area to over-fill. The goal is to restore a gentle convexity, a subtle outward curve, that echoes what was there in youth. Done well, it should be completely undetectable. The aim is never for someone to look as though they have had a procedure; it is for them to look refreshed, healthy, and like themselves.

The injections are placed carefully, with a thorough understanding of the anatomy of the region. The temple contains important blood vessels, and I take great care to work safely around these at all times. In my clinic, I use needles or cannulas depending on the specific area being addressed and the individual patient’s anatomy. Every patient is unique, and the approach is always tailored to every individual.

Treatment typically takes around 20 to 30 minutes and involves minimal downtime. Most patients return to their normal activities the same day, but I advise avoiding exercise and significant heat for the first 24 hours. There may be mild swelling or occasional bruising in the days immediately following treatment, but this settles quickly.

Results are visible immediately, though the filler continues to integrate and settle over the following two weeks, at which point I review every patient. The results typically last between 12 and 18 months, though this varies depending on the individual’s metabolism, lifestyle, and the specific product used.

Who Is a Good Candidate for Temple Filler?

Temple filler is well-suited to patients who have visible hollowing or concavity in the temporal region, whether due to natural ageing, rapid weight loss, intensive exercise, or facial structure. I see it in patients from their onwards, and there is no upper age limit, provided a patient is in good general health.

The treatment may not be appropriate for patients with a history of anaphylaxis or severe allergic reactions, those who are pregnant or breastfeeding, or those with active infection near the treatment area. The best way to assess suitability is always through a consultation. There is no substitute for seeing the face in person and understanding what the patient actually wants to achieve.

Aftercare: What to Expect After Temple Filler

The aftercare for temple filler is straightforward. For the first 24 hours, I ask patients to avoid strenuous exercise, significant heat (saunas, steam rooms, very hot showers), and alcohol. I also recommend sleeping on the back rather than the side to avoid pressure on the temples during the initial settling period, and leaving the area alone — no pressing, massaging, or applying makeup over the injection sites.

Any swelling or bruising, where it occurs, typically resolves within seven to ten days. I review all patients at the two-week mark, at which point the filler has integrated fully, and the results can be properly assessed.

The Wider Impact on the Face

What I find most compelling about temple treatment, and what I think justifies its place at the heart of any upper face rejuvenation plan, is the cascade of improvements it produces elsewhere. Restoring volume to the temples creates a lifting effect on the tail of the brow, which in turn opens up the upper eyelid and the appearance of or hooding. It improves the overall contour of the face when viewed from the side, restoring the smooth, gently curved silhouette that characterises a youthful profile.

For patients who are considering blepharoplasty, surgical eyelid correction, for what appears to be excess upper eyelid skin, I always recommend assessing the temples first. In a meaningful number of cases, restoring temporal volume reduces or eliminates the apparent hooding without the need for surgery at all. That is not to say surgery is never the right answer, but it should not be the first answer when non-surgical options remain unexplored.

Is Temple Hollowing Treatment Right for You?

If your face looks tired despite adequate sleep, if your brows appear to have dropped, if your upper eyelids feel heavier than they once did, or if you simply sense that something has changed without being able to identify exactly what, it is worth having your temples properly assessed.

The best outcomes always come from a thorough, unhurried consultation in which the face is evaluated as a whole, with treatment accordingly. At Health and Aesthetics, that is precisely the approach I take with every patient.
